EXTRA-CURRICULAR ACTIVITIES 

Below is the form for children who attend extra-curricular activities during OOSH session times. 

MEDICAL FORMS 

Asthma Action Plan, Medication record, Anaphylaxis Action Plan. Please click here to see the list of action plans and what is required.

EARLY LEAVERS FORM (BSC ONLY):

If children yr 2+ wish to leave 10mins early at 8:35am to join playground fun before class (supervised by a teacher on duty) they will require a signed permission slip to be excused. This form will be kept on file for future years.
